1) Our body do adjusts to different discomfort and pain levels. The question is how long will it take considering that we are not in a conscious state and our rational thinking cannot influence the process, typically making it shorter. I believe the time we get too frustrated by sleep deprivation and take action comes before the time our subconscious gets adjusted to the new stimuli from the scrotus :D

2) Tight underwear helps because it pushes the cage against the body and counteracts the pushing force of the erection. Less pressure on the scrotum means less stress to its skin.
The type of underwear and of the cage make a big difference thou. I cross-dress and have found out that some female panties and stockings are very good at this job. Might not be easy to fall asleep with all this gear on, particularly if it is your fetish

Btw, I have noted that peeing works to calm night erections. The explanation provided few posts above about the bladder pushing some nerves makes perfect sense to me, very interesting.

Thank you all for the amazing replies.

The real reason I was concerned about whether or not our bodies just get used to the pain or if we stop becoming aroused is because when I wake up my 'bits' are slightly discolored. I love chastity, but it is not worth my health.

Also, it is why I asked about the underwear thing. I just wanted some definite and experienced answers to go off of.

EDIT: I decided to contact the company about purchasing a base ring the next size up and they are sending one out tomorrow. So, hopefully sleep will come easier for the time being.

I definitely recommend a LOT of trial and error for any man who struggles with it. A rare few (perhaps 25%) seem to have no issue from the get-go with it, but most of us find it a real adjustment. It took me years, and ultimately getting a custom made device before I was able to do it easily and w/o any loss of sleep. No combo for the CB 3K worked well enough that I could go without noticing it.

But the 4 main things that I have learned worked for me to help me sleep in lockup:
1) I use a skin lotion at night and not a "lube" like I do during the day. At night = Goldbond medicated, and yeah it *is* menthol, so you will notice it) and during the day I sparingly use A&D ointment.
2) Loose fitting boxer briefs for bed. During the day what I wear varies depending on what I'm doing - a lot of sitting = loose, a lot of walking = tight.
3) Sleep on my side with a pillow under my knee.
4) Be extra mindful about urinating before bed. I will sit on the toilet an extra 10 mins to make sure my bladder is as empty as possible.

Thank you for all the great advice. I can sleep through the night without any issue at all in my CB600s. I am just trying to get to that point with my new metal bird cage and for some reason its just not working out as good. So, though I can use the 1.75" all day without any issue, I have trouble sleeping. The company I bought this from mailed me out a 2" base ring so hopefully I can get used to that one and then downsize again.

I'm assuming you're dealing with mature metal, and they're who I got my custom cage from as well. If you had used a 2" with the 6K I suspect that's exactly what you'll need with the MM cage too. I found the thinner base ring of the MM cage was a nice change from the wider one on the 3K that I used to wear.

If I could sleep semi comfortably with it on I'd probably wear it almost 24/7. Or as long as my gf decided. I just ordered a larger ring myself. Hopefully that'll help.

I find it extremely unlikely that any healthy male simply stops having nocturnal tumescence, given how prevalent it is in males (i.e., nearly 100% of all men have them every single night).

Most likely is that the male genitals adapt - the skin of the scrotum elongates to enable erections to occur without testicular pain, or pinching pain due to skin pulling.

Keep in mind of course that at different times of year, skin will be more or less elastic, based on temperature and humidity both at night and during the day. I myself had a problem just the other night because it is below 20 degrees F outside here, and my testicles were very tight due to the cold.

Psychological changes from long term chastity aside, it is important to keep in mind that erections, sex, and ejaculation are literally the only real reason for the biological entity that is a human male, just as nurturing the maternal cell line within a woman's ovaries and generating the next generation is the purpose of the human female. We're both big biological machines to make gametes meet each other. You won't defeat that with a small piece of steel, unless you're causing structural damage.

Balmy! Is Spring coming already?

Well below zero here last night...and one of my heaters is [temporarily] out of commission.
