While I can't say for sure how it would be with the Real Thing, I can certainly say that having something that *looks* like a real cock and *feels* like a real cock in one's mouth, and still sporting a hard-on, can be somewhat...embarrassing...and might be even more so if she were to notice and use it as an excuse for further embarrassment. In my mind, the effect of the mental and/or physical 'simulation' is not so very far away from the reality, just one tiny little step...but maybe a big difference? Yes, which is why it is there. I thought of leaving it off but if all that the guy is doing, he would have no option to select.
