Nothing works better for me than a decent sized dildo with some sort of texture (ridges, big head, etc). Well, nothing other than my owner’s skilled fingers. Massagers haven’t worked for me - vibrating or not.

Yes 5 mm is a huge difference. After a few months in a cheap cage, I started in a 1 3/4” (45 mm) base ring in my custom cage. Roughly six months later, it was far too big. Moved to 1 5/8” (42 mm) and it’s a perfect fit.

Slipping out is a function of the ring size, not cage length. Look for something around 42 mm. Cherry Keeper offers more sizes and are inexpensive.

Some testicular soreness is normal, especially early on and after a night of frustration. Could be caused by the cage pulling away and applying pressure, or just blue balls. Excellent points and duly noted. I hadn’t planned on it being used for more than a few hours when work requires. I think my owner is enjoying how much more restrictive it is.

Side note: cold temperatures may make your base ring tighter, as the scrotum pulls up. The difference, for me anyway, can be substantial. We have my old larger ring ready for winter.
