For the CB-x000 devices, I believe you measure from the top (base of your penis). Steelworxx also has you measure the full length from the base.

I have seen the MM way of measuring referenced on the forum many times and apologize for the misconception.

My questions are; when you measure this way how do you determine the base? Do you push a ruler in slightly? If you have extra skin from being overweight, or how do you account for that?

Just lift up and lay it down - if you push in you'll end up with a cage that's too long - i'e imagine where the end of the cage would fit at the base of the penis.
