I have no experience with this type of belt. However, the problems you have with your current device are pretty common and there are things you can do.

Using a bit of lotion, lubricant, or powder on the skin around the base ring helps a lot. I very quickly became a fan of a product called "Fresh Balls," which is a lotion that dries to a powder, for this purpose.

The above trick helps a lot with nighttime erections, too. I find a lot of the pain from an erection is because the pressure increases friction on the skin. It will also become easier with time. My experience is that you need about 3 or 4 days in the device for your penis to start learning it can't get so hard anymore, and I've read that this is most other men's experience. In the mean time, try getting up to pee when it gets too hard. Hard penises tend to get soft when you pee.

As far as noticability under clothing: You might be overestimating it. I did, and I think most people do to begin with. Most people probably don't pay attention and won't notice unless it's inhuman in size or shape. Even if it is a problem, I imagine that getting a custom-fit device might be a better solution than switching to a different type of device. I don't know about the bulge specifically, but it seems like a full belt is much, much more hardware to keep hidden than a ball-trap device.

Two things:
1. Any belt requires custom fitting. There is no way something you order from China will fit correctly. I had an Access Denied belt and it took two fittings before it was right. it is also difficult if you gain or lose weight. A properly fitted belt only works if you stay the same size. The fact that belts are so tough to fit is why cages are so popular. Once it was correctly altered, the Access Denied belt did restrict movement a bit, but was ok to wear. I had no access at all to my gentials.

2. The problems you are having with your cage are probably a too-small cock ring. I have no pain when I try to get hard and am barely aware I am wearing a cage. I have mine on 24/7.

The belt sizing won't be quite as critical, and the section that goes behind the testicles could be left looser for comfort. And I'm thinking that somebody handy could make the waist belt smaller if he loses weight. The belt could probably be modified to fit more conventional devices, too.

My experience with the Access Denied belt is that it took almost a full week of 24/7 wearing to get used to it. In terms of fit, you will see how the adjustments work for you. It's a big investment. You may want to consider a cage first. If you have a well-fitting belt, it is a cool feeling in public. Bear in mind that if you add the butt plug, your wear time will be, at most, only a couple of hours.
