Can this really prevent me for having intercourse with other women?
Is there any risk about making any damage in my penis?
Should I tell her another device?

I guess for the purpose of not having sex with other women a padlock through the PA could work quite well. You could gauge up to a decent size lock. What guage size is 5/16"?

Still be able to urinate and even masturbate without trouble. Certainly couldn't have PIV sex though.
You would want to devise some sort of support to keep the weight from pulling on you.
