Second the S87 type. I have a Chinese version of this, with the urethral tube. I don't use the tube, and the design is very comfortable. The tip of your penis doesn't extend out and rub on your shorts like it might with a more open front design. Urinating is a little more messy, but not too bad.

Actually the 8mm size isn't uncomfortable or hard to use. Your urethra is bigger than you think.
Besides a piercing, I think a tube is the only way to be as close to 100% secure as you can get.
You might be able to pull out using a tube, but you'd never be able to get back in.
