Another problem I have sometimes, is that when wearing my cage, the balls are in a different position (than without cage), and they can rub on my thighs during walking. And it causes irritation to the scrotum.
What can be done against this?

I made some adjustements to my cage, by inserting folded toilet paper between shaft tip the cage tip, I can reduce the cage length without exiting the cage.
Added to allowing more time from cage exit to PIV. I suffered no adverse effect today...
